The Southport Inquiry has published its phase one report examining the roles and actions of policing, Prevent, education, social care and healthcare in the handling of AR before the 29 July 2024 attack. The report identifies systemic failures, including an absence of risk ownership, critical failures in information sharing, a misunderstanding of autism which contributed to inaction, a lack of oversight of online activity despite clear indicators of violent preoccupation and parental failures. It makes recommendations on cross‑border information sharing, record keeping, training, risk assessment and multi‑agency coordination, noting that February 2026 guidance on child knife possession now requires mandatory Youth Justice Service notification and structured intervention.
