A. Additional documents for main applicant

Evidence of endorsement from the sport governing body
Evidence for maintenance
Any combination of A, B or C should be provided. All should cover a consecutive 90-day period ending no more than 31 days prior to the application.
A. Personal bank or building society statements. These should be either:
—bank stationery sent in the post
—ad hoc statements printed on the bank's letterhead (excluding mini-statements from Automatic Teller Machines (ATMs))
—electronic statements plus a supporting letter from the bank on company-headed paper verifying their authenticity, or
—electronic statements with the official stamp of the bank on each page
All statements should include the following details:
—your name
—your account number
—date of each statement
—the financial institution’s name and logo, and
—any transactions over the period
B. Building society pass book
This should include:
—your name
—your account number
—the financial institution’s name and logo, and
—any transactions over the period
C. Letter from the bank, building society or financial institution regulated for the purpose of personal savings accounts by the Financial Conduct Authority and the Prudential Regulation Authority, or the home regulator in the case of a financial institution operating outside the UK
This must be prepared on the institution's own letterhead paper and include:
—your name
—your account number
—date of the letter (which should be no earlier than 31 days before the date of application)
—the financial institution’s name and logo
—the current funds in
