the hours the employee is required to work (for example a request to work fewer hours per week); and/or
the times when the employee is required to work (for example starting earlier or later in the day); and/or
the location (any place of business of the Company and/or at the employee’s home) at which the employee is required to work (for example, a request might be made by an office-based employee to work one day a week from home).
