Checklist and record—clearance to deal request by PDMR or director

Using this checklist and record:

On receipt of a clearance to deal request from a person discharging managerial responsibility (PDMR), a person closely associated with a PDMR (PCA) ie family members, legal persons, trusts or partnerships or a director of the company, this form should be completed and kept with the original clearance to deal request (and any supporting documentation). If it is not possible to retain the original clearance to deal request a copy should be taken and attached to this record instead.

Once the clearance to deal request has been considered by the appropriate person and a decision has been made as to whether to give clearance to deal or not a copy of the response and clearance (if given) must be provided to the PDMR, PCA or director concerned.

The response should be provided with five business days of the clearance to deal request being received.

Where clearance to deal is given, follow up with the PDMR or director to confirm the action taken by the PDMR or director and ensure that it was in accordance with the clearance to deal provided, in particular note that:
