In contrast to the open-ended nature of requests that may be made, the provisions as to the form of an application are highly prescriptive. This does not prevent employers from dealing pragmatically with applications not conforming to the statutory requirements, and it is likely that, as with flexible working requests, the full formalities will rarely be insisted on. However employers minded to do so may insist on strict compliance with the formalities, and refuse to consider applications not in the correct form or not containing all the required information.
