An application for a criminal record certificate1, an enhanced criminal record certificate2 or a criminal conviction certificate3, and any associated request4, must be made in a form and manner determined by DBS5. In order for a certificate to be issued an applicant must be aged 16 or over6. Evidence of identity is also required7.
