The trustees or managers1 of any scheme2 must furnish in writing:
(1) as of course to any person as soon as practicable and, in any event, within two months after he or his employer3 has notified the trustees that his pensionable service4 has terminated; and
(2) to any member or prospective member5 on request (not being a request made less than 12 months after the last occasion on which such information was furnished to him) as soon as practicable and, in any event, within two months after he requests it,
information as to the rights and options (if any) available
