If:
(1) a complaint is made to the Secretary of State or, in relation to Wales, the Welsh Ministers1 that any library authority2 has failed to carry out duties relating to the public library service imposed on it by or under the Public Libraries and Museums Act 19643; or
(2) the Secretary of State or the Welsh Ministers is or are of opinion that an investigation ought to be made as to whether any such failure by a library authority has occurred4,
and, after causing a local inquiry5 to be held into the matter, the Secretary of State or the
