A care and treatment plan1 may be reviewed or revised by the care coordinator at any time provided that the care coordinator agrees to that review or revision2.
A care coordinator must review and, if necessary revise, a care and treatment plan3 when:
(1) a period of no more than twelve calendar months has elapsed since the initial preparation or the last review of that plan4;
(2) a relevant patient5 requests a review of his or her plan before the twelve calendar month period has elapsed6;
(3) a relevant patient's carer or adult placement carer7 requests a review of that
