An individual who is discharged from secondary mental health services must be provided in writing with the reason for his discharge and, if that individual considers that further support and advice in relation to his mental health is required following discharge, the action which may be taken, and by whom1.
An adult must be provided with information in writing regarding his entitlement to an assessment2 of former users of secondary mental health services3.
Where an individual is discharged from secondary mental health services as a child but becomes an adult during the relevant discharge period the following information must be provided
