A relevant patient's care coordinator1 must work with the relevant patient and the patient's mental health service providers2:
(1) with a view to agreeing the outcomes which the provision of mental health services3 for the patient are designed to achieve, including (but not limited to) achievements in one or more of the following areas:
(a) finance and money4;
(c) personal care and physical well-being6;
(d) education and training7;
(e) work and occupation8;
(f) parenting or caring relationships9;
(g) social, cultural or spiritual10;
(h) medical and other forms of treatment including psychological interventions11;
(2) with a view to
