For the purpose of improving the effectiveness of the mental health services provided to a relevant patient1, a mental health service provider2 must take all reasonable steps to ensure that:
(1) different mental health services which it is responsible for providing for the patient are coordinated with each other3;
(2) the mental health services which it is responsible for providing for the patient are coordinated with any other such services the provision of which is the responsibility of any other mental health service provider
