The authority for transfer from guardianship to hospital1 must be given in the prescribed form2 by the responsible local social services authority3. No such authority may be given unless: (1) an application for admission for treatment4 has been made by an approved mental health professional5 in the prescribed form6; (2) that application is founded upon two medical recommendations or a joint medical recommendation in the prescribed forms7; (3) the responsible local social service authority is satisfied that arrangements have been made for the admission of the patient to that hospital8; (4) in relation to Wales, an application for admission
