A person whose name has been erased from a register under a direction of the fitness to practise committee1 or who has had an entry relating to a specialty or proficiency removed under a direction of the fitness to practise committee2 (otherwise than for a specified period) may apply to the registrar3 for his name, or his entry relating to a specialty or proficiency, to be restored to the appropriate register4.
