The following apply to an application for restoration to the register made by a person ('the applicant') who was previously registered and whose name was erased from the register for any reason1.
Such an application must be accompanied by:
(1) evidence that, in relation to the most recent CPD cycle completed by the applicant, the applicant met (where applicable) the requirements relating to CPD, the CPD record and the annual CPD statement2
(2) a record of all CPD undertaken in the shorter of the period:
(a) beginning with the end of the CPD cycle to which the evidence submitted
