The registrar1 may remove a registrant's name from the appropriate register if he is not satisfied that:
(1) he has received from the registrant by the relevant date2 a fully completed application form3 for retention of registration4;
(2) the registrant is entitled5 to be registered6;
(3) the registrant is covered by a policy of insurance which meets the insurance requirements7 for individual registrants and persons applying for their names to be registered8; or
(4) the application fee has been paid9.
The registrar may remove the entry of an independent prescribing specialty against the name
