The right to registration in the register1 is conditional on the making of an application supported by the following evidence2. Any person applying to be registered in the register must produce or send to the registrar3 the document conferring or evidencing his licence or other qualification, a statement of his name and address and the other particulars (if any) required for registration and the documents conferring, or evidencing that there is in force in relation to him or there will be4 appropriate cover under an indemnity arrangement
