A person applying for full or provisional registration as a medical practitioner1 or for registration of any primary United Kingdom or primary European qualifications2 he holds3 must produce or send to the appropriate registrar4 the document conferring or evidencing the qualification5 by virtue of which the application is made, together with a statement of the applicant's name and address and such other particulars, if any, as may be required for registration6.
However, any body or combination of bodies entitled to grant primary United Kingdom qualifications for these purposes7 may, from time to time, send to the registrar8 or the registrar
