An application for admission to a part of the register1 must be: (1) made in writing2 using the personalised documentation provided by the Nursing and Midwifery Council3 which must include a declaration by the applicant as to his good health and good character and the other specified information4; (2) signed by the applicant5; (3) sent or delivered to the registrar6; (4) supported by payment of the appropriate fee7; (5) accompanied by evidence of his qualification8 and a supporting declaration as to his good health and good character9.
