A person may have access to any medical report1 relating to him which is to be, or has been, supplied by a medical practitioner2 for employment3 or insurance4 purposes5. A person may not apply to a medical practitioner for a medical report to be supplied to him for employment or insurance purposes unless he has notified the individual to whom the report relates of his intention to make the application6 and the individual has notified the applicant that he consents to the making of the application7.
