A tenant's notice1 of his desire to have the freehold or an extended lease of a house2 and premises3 must be in the prescribed form4, and must contain the following particulars5:
(1) the address of the house, and sufficient particulars of the house and premises to identify the property to which the claim extends6;
(a) such particulars of the tenancy7 and, in the case of a tenancy entered into before 1 April 1990, or on or after 1 April 1990 in pursuance of a contract made before that date, and where the property had a rateable value8
