An application may be made to the appropriate tribunal1 for a determination whether a service charge2 is payable and, if it is, as to3:
(1) the person by whom it is payable4;
(2) the person to whom it is payable5;
(3) the amount which is payable6;
(4) the date at or by which it is payable7; and
(5) the manner in which it is payable8.
This applies whether or not any payment has been made (and, accordingly, payment by the tenant does not mean that he or she agrees with the charges)9.
