Where any written demand1 is given to a tenant2 of premises which consist of or include a dwelling3, and are not held under a tenancy to which Part II of the Landlord and Tenant Act 19544 applies, the demand must contain the following information, namely:
(1) the name and address of the landlord5; and
(2) if that address is not in England and Wales, an address in England and Wales at which notices, including notices in proceedings6, may be served on the landlord by the tenant7.
