A person making a thorough examination1 for an employer2 must:
(1) notify the employer forthwith of any defect in a power press3 or its guard or protection device which in his opinion is or could become a danger to persons4;
(2) as soon as is practicable5 make a report of the thorough examination to the employer in writing authenticated by him or on his behalf by signature or equally secure means and containing the specified information6; and
