376. Recording and record-keeping in regard to injuries, disease, etc.

The responsible person1 must keep a record of any:


    (1)     reportable incident2 in relation non-fatal injuries to workers or non-workers, work-related fatalities or dangerous occurrences3, which contains the specified particulars4;


    (2)     diagnosis5 reportable in relation to occupational diseases, exposure to carcinogens, mutagens and biological agents or diseases offshore6, which contains the specified particulars7;


    (3)     injury to a person at work8 resulting from an accident9 arising out of or in connection with that work, incapacitating that person for routine work10 for more than three consecutive days11 (excluding the day of the accident), which contains the specified particulars12; and


    (4)     other
