Every employer1 must ensure that each of his employees who is or is liable to be exposed to lead2 is under suitable medical surveillance3 by a relevant doctor4 where:
(1) the exposure of the employee to lead is, or is liable to be, significant5;
(2) the blood-lead concentration or urinary lead concentration of the employee is measured and equals or exceeds the specified levels6; or
(3) a relevant doctor certifies that the employee should be under such medical surveillance,
