Where the age of any person is required to be ascertained or proved for the purposes of the Factories Act 1961 any person is entitled, on presenting a written requisition in the prescribed form1, and on payment of the appropriate fee2, to obtain a certified extract, under the hand of a registrar or superintendent registrar, of the entry in the register under the Births and Deaths Registration Act 1953 of the birth of that person3. The form of requisition must be supplied on request without charge by every superintendent registrar
