Where required to follow the reporting procedure1 in relation to non-fatal injuries to workers or non-workers, work-related fatalities or dangerous occurrences2 in the case of a mine3 or quarry4, the responsible person5 must: (1) notify the relevant enforcing authority6 and any nominated person7 of the reportable incident8 by the quickest practicable means without delay; and (2) send a report of that incident in an approved manner9: (a) to any nominated person within seven days of the incident; and (b) to the relevant enforcing authority within ten days of the incident10.
