Where it is proposed that the work to which an initial notice1 relates should be varied2, if:
(1) a notice in the prescribed3 form (an 'amendment notice')4 is given to the local authority5 by whom the initial notice was accepted, and is jointly given by the approved inspector6 who gave the initial notice7 and by the person shown in the amendment notice as the person intending to carry out the relevant work8;
