Where a debtor1, being an individual:
(1) intends to make a proposal for a voluntary arrangement2, but an interim order has not been made3 in relation to the proposal and no application for such an order is pending; and
(2) if he is an undischarged bankrupt4, has given notice of the proposal to the official receiver5 and, if there is one, the trustee6 of his estate7,
then the debtor must, for the purpose of enabling the nominee8 to prepare a report9, submit to the nominee:
(a) a document setting out the terms of the voluntary arrangement which the debtor is
