The official receiver1 may revoke or amend a debt relief order2 during the applicable moratorium period3 on the ground that:
(1) any information supplied to the official receiver by the debtor4 in, or in support of, the application5, or after the determination date6, was incomplete, incorrect, or otherwise misleading7;
(2) the debtor has failed to comply with the duty to provide assistance to the official receiver8;
(3) a bankruptcy order9 has been made in relation to the debtor10;
(4) the debtor has made a proposal for an individual voluntary arrangement11, or has notified the official receiver of his intention
