A creditor or a member state liquidator1 appointed in main proceedings in relation to the debtor who intends to appear on the hearing of the petition must deliver a notice of intention to appear to the petitioner2. The notice must contain the following:
(1) the name and address of the person, and any telephone number and reference which may be required for communication with that creditor or with any other person, to be also specified in the notice, authorised to speak or act on the person's behalf;
