An application for a debt relief order1 must be made to the official receiver2 through an approved intermediary3 and must include:
(1) a list of the debts to which the debtor4 is subject at the date of the application5, specifying the amount of each debt6 including any interest, penalty or other sum that has become payable in relation to that debt on or before that date, and the creditor to whom it is owed7;
(2) details of any security held in respect of any of those debts8;
(3) the following information:
