When an inspector1 has completed an investigation into an accident2 or incident3, he must prepare a report of the investigation in a form appropriate to the type and seriousness of the accident or incident4.
The report of an investigation into an accident must state that the sole objective of the investigation is the prevention of accidents and incidents5; and, where appropriate, the report must contain safety recommendations6. The report of an investigation into an incident must, where appropriate, contain relevant safety recommendations, protect the anonymity of the persons involved in the incident, and be circulated by the investigating inspector7 to
