345.     Keeping of records and confidentiality.

Every licence holder1 must record, as soon as reasonably practicable, the following details in relation to every application received from a worker2:

  1.  

    (1)     the date terms are agreed between the licence holder and the worker3;

  2.  

    (2)     the worker's name, address and, if under 22, date of birth4;

  3.  

    (3)     any terms which apply or will apply between the licence holder and the worker, and any document recording any variation5;

    (4)     any relevant details of the worker's training, experience or qualifications and any authorisation to undertake particular work (and copies of any documentary
