The procedure for the execution of an instrument intending to create a lasting power of attorney1 is:
(1) the donor must read (or have read to him) all the prescribed information2;
(2) the donor must complete the provisions of the instrument3 that apply to him (or direct another person to do so4) and sign5 the relevant part6 in the presence of a witness7;
(3) the person or persons giving an LPA certificate8 must complete9 and sign it10;
(4) the donee
