5 Enrolment of deed poll evidencing change of child’s name

Where a change of name of a child is to be evidenced by enrolment of a deed poll1, special provisions as to execution and contents are prescribed2. If the child is under the age of 16 years, the deed poll must be executed by a person with parental responsibility for the child3. If the child has attained the age of 16 years the deed poll must, except in specified circumstances4, be executed by a person having parental responsibility for the child and be endorsed with the child’s consent signed in
