4.10 Requests to amend formal records

Where holders of formal records, such as schools, doctors and local authorities, are requested to amend their records so as to give effect to a change in a child’s name, they should first enquire whether any person other than the person making the request has parental responsibility. If there are other such persons, the school, doctor or other body should only record the change if satisfied that all persons who have parental responsibility consent to the change, or that there is a court order giving leave for the change1.
